4. Std. 12 (HSC) mark sheets of all attempts and passing certificate.
5. School Leaving Certificate (2 copies).
6. G.N.M. mark sheets of all years and all attempts.
7. B.Sc. (Nursing) mark sheets of all years and all attempts.
8. Registered Nurse / Registered Midwife (G.N.C. Registration) certificate.
9. Proof of Gujarat Nursing Council registration status – whether permanent or temporary; if temporary, valid-until date and all renewal receipts, or the permanent registration receipt.
10. Computer Basic Knowledge certificate as per the General Administration Department circular dated 13-08-2008.
11. For Scheduled Caste (SC) category candidates: caste certificate issued by the competent authority. Additionally, for caste verification, a separately downloaded and filled caste-verification form (as per the linked format) along with the required supporting documents must be submitted.
12. For Socially and Educationally Backward Class (SEBC) candidates: the prescribed non-inclusion (non-creamy layer) certificate format, with the certificate number and date as mentioned in the online application, valid as on the date of advertisement.
13. For Economically Weaker Section (EWS) candidates: the prescribed EWS certificate format.
14. For candidates with physical disability: a Superintendent/Civil Surgeon medical certificate as prescribed under the General Administration Department circular no. Parcha/102008/469540/G-2 dated 01-12-2008.
15. For Ex-Servicemen: original Discharge Book and certificate/identity card issued by the concerned department, along with original certificates of a minimum 5 years of military service including E.C.O./S.S.C.O. service, plus attested copies of each.
16. For government employees: a letter confirming that the concerned department/office was informed within 7 days of applying online, plus a “No Objection Certificate” from the office, original and attested photocopies.
17. If the candidate’s name has been changed, supporting proof of the name change.

Additional Verification Instructions

• Candidates must have completed Gujarat Nursing Council registration at the time of applying (after passing B.Sc. Nursing / G.N.M.); the certificate bearing the registration number mentioned in the application must be produced.

• A photo ID proof (Driving Licence, Voter ID, Aadhaar Card, or Passport) must be brought in original along with one attested copy.
• Married female candidates whose name differs between the application and certificates must bring the original and an attested copy of the Marriage Registration Certificate / Government Gazette notification.
• The photograph uploaded on the OJAS online application printout must match the photograph the candidate brings; the candidate’s signature must also match the one on the application printout.
• Any discrepancy found between submitted documents and the online application details will be decided by the Recruitment Committee, and the decision will be final and binding on the candidate.
