• The “Brief bio-data of candidate” form (Non-Teaching), printed with the notice. It asks for:

  • a photo;
  • your full name, gender, date of birth and category (OPEN / SC / ST / SEBC / PH);
  • your address for communication, telephone number, mobile number and email ID;
  • your qualifications (degree or diploma, university or institution, main subjects, class, percentage of marks and year of passing), including M.Phil., Ph.D. and any other qualifications or certificates;
  • whether you have cleared NET or GSET for Asst. Prof;
  • your experience, starting with your current job (employer, designation, nature of responsibilities, period from and to, and gross salary). Under nature of responsibilities, mention any undergraduate or postgraduate teaching experience. Attach an extra sheet if you need more space.
• Self-attested copies of all documents.
• Your signature on the form.
